Forms

Circulation Forms 

All forms that you may need can be found in the drawer of the Circulation Desk.  Forms include: proxy borrower authorization form, application form for patrons, pins for the visitor computers, library staff and faculty business cards, as well as visitor computer pins.

Proxy Borrower Authorization Form 

If someone would like to check out items on behalf of another patron, we must have a Proxy Borrower Authorization form on file. The most common situation is that a faculty member would like for their student employee to pick up books on their behalf. Due to confidentiality and privacy reasons, we are not able to allow anyone to check out items on behalf of other patrons unless we have that form on file. 

There is a physical form on the shelf, but you can also email them the online form.

Application for Library Patrons 

Anyone who is not a current GU faculty, staff, or student must fill out an application form to be added into Alma.  

Library Faculty & Staff Business Cards 

You can feel free to hand out business cards to patrons who may have questions. Situations that might call for you to hand out a business card include: you have referred someone to a librarian and want to give them the correct contact information; a patron has a complaint and would like to speak to someone in library admin (you could give them Laura, Brad, or Heather’s card).  The cards can be found in in the circulation desk drawer. There is one card holder for librarians and one for staff and admin. The cards in both card holders are filed by first name.

Visitor Computers and Pins 

Visitors are allowed to use one of our four designated visitor computers during established visitor hours. They must sign in using the visitor computer log at the service desk. Pins for the computers can be found in the circulation desk drawer.

Library Incident Form 

There is a tab in the Chrome browsers on the circ computers that is titled "Library Incident Form." This form is for reporting incidents where security has been called or there is another reason that library administration should know about the incident. If you are present during such an incident, a supervisor will ask you to fill it out and to report, to the best of your knowledge, the details of the incident. Do not fill out this form unless a supervisor or lead student has asked you to do so.
